Appointments:

All private client appointments will be arranged on an individual basis by Cindy Silver, Registered Dietitian, LLC. A reminder of the appointment will be emailed to you 48 -72 hours before your appointment date/time, with confirmation requested. Please email your confirmation promptly. If you need to reschedule or cancel, please inform Cindy Silver, Registered Dietitian, LLC. by email or phone at least 48 hours prior to your scheduled appointment.

Payment:

If you are a private pay client then full payment will be due at the time of your nutritional counseling appointment.

Privacy:

Every reasonable effort of security will be made by Cindy Silver, Registered Dietitian, LLC to protect your privacy. No client information is collected on this website. Any information that you do provide will only be accessed by Cindy Silver, Registered Dietitian, LLC. Throughout your nutritional counseling, strict privacy will protect your name, phone number(s), email address(es) and message content, and your health information. Prior to your first nutrition appointment, a Notice of Privacy Practices from Cindy Silver, Registered Dietitian, LLC will be shared with you. Or you may choose to view it now in the Services drop down menu under ‘Privacy Practices.’

All client personal information and records are confidential and will not be shared with any third party. All client conversations with Cindy Silver, Registered Dietitian, LLC are confidential under HIPAA – Health Insurance Portability and Accountability Act.
